Infertility Treatment
Infertility treatment is a limited benefit. Diagnostic services for infertility testing are a covered benefit to identify physical illnesses or injuries to the reproductive system, such as semen analysis, hormone evaluation, chromosomal studies and immunologic studies. Treatment for infertility for men and women may include the following depending on the cause:
- Hormonal treatment
- Corrective surgery
- Administration of Human Chorionic Gonadotropin (HCG)
- Antibiotics
- Radiation therapy
Cost Information
Artificial insemination or any other forms of artificial conception including services, supplies and drugs related to these procedures, are not a covered benefit. This includes:
- In Vitro Fertilization (IVF)
- Intrauterine Insemination (IUI)
- Gamete Intrafallopian Transfer (GIFT)
- Zygote Intrafallopian Transfer (ZIFT)
- Reversal of surgical sterilization
