Privacy Act Statement
A Privacy Act Statement must be provided to TRICARE beneficiaries when soliciting or collecting personally identifiable information (PII) for inclusion in a system of records.
Notice of Privacy Practices
The Notice of Privacy Practices describes how HNFS may use and disclose protected health information. It provides information about your rights to access protected health information and to request amendments, restrictions or an accounting of disclosures of protected health information.
The Notice of Privacy Practices also describes the procedures for filing a complaint. As discussed in the complaint section, you may file a written complaint in accordance with the Notice of Privacy Practices or with the Department of Health and Human Resources.
No retaliation will occur against you for filing a complaint.
The Defense Health Agency and Department of Veterans Affairs are responsible for the maintenance of their respective Notice of Privacy Practices.
