This program is divided into modules (topics) which are self-guided or video presentations. The video presentations include audio. For most of the topics, we also offer related resources, interactive worksheets, educational handouts, and web links. You can access the related resources while viewing the module presentations or at any time by visiting this page. Once you have completed a module, simply close the window to return to this main page.

Topic presentations may take a few minutes to load.

Program Directions

1.  Complete the presentation slides in each module under the Module Topic column in the table below.

a. Click on the Module Topic, then click "Enter" to get started.

b. Use the yellow arrows in the top right of each presentation to go to next and previous screens.

c. Use the volume control in the lower right corner to lower or turn off volume. You may also need to use your personal device for volume control. For videos, you will need to use your personal device to control volume.

d. You can use the pause and play button within each slide to toggle back to the Related Resources section in your bowser at any time.

e. Once you've completed the presentation (or a video), you can close it by clicking on the X in the browser. The HW4L Resources page should still be open in a separate tab of your browser, so you can return to complete Related Resources, or go to the next module.

2.  Complete the related forms and/or links under Related Resources in the table below.

3.  Complete the program evaluation located in the Related Resources for Module 7. Whether you take the entire program or only a portion, your feedback is greatly appreciated! Once you have completed your evaluation, you can print a personalized Certificate of Completion.

Program Tips

Create a folder on your computer where you can download and save all your completed forms. They will be valuable for you to print and/or review after completing the program.

Do More to Improve Your Health

If you would like individualized education and support, you may request to be in our Chronic Care/Disease Management program, which provides individual coaching for certain health conditions. If you are an eligible TRICARE beneficiary under age 65, receive one-to-one coaching from a Disease Management Specialist. Get started with our Disease Management Information and Self-Nomination form, or you can call 1-844-732-2436 from 8:00 a.m.–6:00 p.m. local time, Monday through Friday.
